OpenRoster Athletics Waiver & Release Agreement

Participation in OpenRoster Athletics programs and activities involves inherent risks associated with youth sports and physical activity, including the possibility of injury, illness, or accident.

By registering, I acknowledge that participation is voluntary and that I assume all risks associated with participation on behalf of myself and the participant.

I release and hold harmless OpenRoster Athletics, its coaches, staff, volunteers, representatives, facility partners, and affiliated organizations from claims, liabilities, damages, or expenses arising from participation in OpenRoster Athletics activities, to the fullest extent permitted by law.

I authorize OpenRoster Athletics to obtain emergency medical care for the participant if necessary and understand that I am responsible for any related medical expenses.

I also grant permission for OpenRoster Athletics to use photographs or video of the participant for promotional, marketing, website, and program-related purposes unless otherwise requested in writing.

By submitting this registration, I certify that I am the parent or legal guardian of the participant and that I have read and agree to this Waiver & Release Agreement.
